New client onboarding

We use another system called therapy appointment for scheduling and keeping in contact with our clients. 

Please fill out the form below, and I'll add you into the appointment system. 

For new patients:

Fill out all the fields, and I can add you into the system as soon as possible. 

I normally have it done within 24 hours.

You've received an email from Therapy appointment?

To register as a client, please click the link emailed to you from Therapy Appointment. Enter your birthdate, create a username and password, agree to the terms, and complete the required personal and insurance information.

Once registered, you’ll be directed to the client portal homepage. Click “Docs and Forms” on the left to complete and E-sign the necessary forms before your first session. 

 

One thing that I want to make sure you notice before your first appointment is that in the “Informed Consent” section of the paperwork, it does inform you that you are responsible for knowing the limits and specifics of your insurance. Therefore, it may be a good idea to call your insurance company before your appointment, otherwise you will be required to pay the full amount for your session (you will be reimbursed if and when payment from your insurance company is received)

Client portal

You can access the client portal anytime to schedule appointments, make payments, update information and launch telehealth sessions.
